David Sinclair's Age-Reversal Trial: What It Actually Tests (And Does Not)

Age reversal: David Sinclair's company has dosed the first human with epigenetic reprogramming. The trial is real, it is first-in-human, and its primary endpoint is safety.

On January 15, 2026 the FDA authorized it. On June 9 a patient received an injection into one eye, carrying instructions to partially reset the age of the cells inside it. It is the first time partial epigenetic reprogramming has been tried in a human being.

So what do we actually know? Less than the headlines suggest, and more than you would think.

The trial is ER-100, from Life Biosciences, co-founded by Harvard geneticist David Sinclair. He is the principal in this story, not a commentator on it, and this video says so up front. It enrolls 18 people. It lists 21 primary outcome measures, and every one of them is safety. It is not designed to answer whether age reversal works.

This walks what actually happened, in order: the 2020 Nature mouse result, the primate data, the FDA authorization, the first dose. Then what the registry actually commits to measuring. Then the part worth knowing: why they used 3 of the 4 Yamanaka factors instead of all 4, and why the therapy only runs while the patient takes an antibiotic. The most interesting thing in the first human age-reversal trial is not the reprogramming. It is the off switch.
